Gift Form:
This
form states that the donor is gifting his/her body to the WWAMI
Medical Education Program. Please make copies for your files and
the person in charge of your affairs upon your death. Please do
not forget to sign and date this form. This form needs to be signed
and dated by two witnesses as well.
Donor
History Form:
This form provides our Program with important contact information about you and your survivors, information that allows us to effectively carry out our anatomical studies, and important data that is required by our funeral directors in order to complete the death certificate.
Final Interment Form:
Please
choose one of the options on this form. The options include:
- Burial in the
WWAMI Medical Education Program Plot at Greenwood Cemetery,
Palouse, Washington. - The return of your cremated remains to a funeral director of your choice for private arrangements. Please fill out this form completely and do not forget your signature and the date.
